Pick Elementary is deeply saddened at the passing of our dear friend and coworker, Amy Core. Amy died unexpectedly after an emergency surgery on July 18, 2014. Amy helped open Pick Elementary in 2013 and taught 4th graders. Prior to that, she worked at Richland Elementary, Cary Woods Elementary and Auburn Early Education Center. We extend our most heartfelt sympathies to her family.
Medication Information -
-2014-2015 School Year
If your child will require medication during the 2014-2015 School year, you must contact the school nurse. A completed Medication Parent/Prescriber Authorization form must be submitted when the medication is delivered to the school nurse. Students are prohibited from delivering medications to school.
All Students- During the 2014-2015 School Year, the Alabama Department of Public Health will conduct an audit to ensure compliance with immunization requirements. If you choose to have your child’s records be exempt from the audit, you must submit this written request to the school nurse.
Health Assessment Record Forms
Parents must complete and submit a separate Health Assessment Record form for each child. A new form is required to be submitted directly to the school nurse each school year. Forms should be submitted before the beginning of the school year.
Every child who is diagnosed with a chronic condition requiring health services in the school setting should see a medical provider as soon as possible and obtain new medical orders for the upcoming school year. New orders must be submitted for each school year prior to the start of school. Parents may pick up the required health packets from the school nurse so that all forms may be completed at the medical visit and signed by the provider. After the medical visit, parents must schedule a conference with the school nurse in order to develop the child’s health plan.
