Any student who rides a bus to a school-sponsored activity must ride the same bus back to Auburn unless this form is completed and returned to Dr. Logan.

A student will be released ONLY to:

1. An adult relative

2. The parents of another member of the Auburn High School Band

This form must be completed and returned to Dr. Logan as far ahead of the trip as possible. Dr. Logan will contact one of the parents, if needed, for verbal verification.
