You may sign up by
mailing forms you have been sent or dropping them off in Dr. Long's office.
Also, there are extra forms in the office if you need one.
You may also sign up and
register your guest and pay during lunch waves April 9th,10th, and 11th.
The FINAL DEADLINE to sign up, register and be paid in full is April 20.
Ticket pick-up will be during lunch waves on May 1st and 2nd.
