to take this survey as either an administrator or teacher,
you first must register accordingly below.

this, click on the correct link in the "[Step 1]: Survey
Registration" column below

You will
be moved to a new window where you will select
Auburn City.

You will
then be directed to a Teacher Registration & Login Creation
window.
This will open a new window in which you will be asked to
provide information necessary to create your login id.

Once you
have submitted this information you will be given an
opportunity to take the survey
or
you can save your login and come back (to this page) to
complete a survey at a later time by clicking on the
appropriate link under the "Login & Take Survey" column
below.
